Home Care Coordinator Registered Nurse

The Role

We are seeking a caring, reliable, dynamic, competent and capable Home Care Coordinator Registered Nurse based in the CLEVE OR COWELL full time or part time. 

Role Duties

  • To support people to make informed choices about their aged care or disability support services.

  • To partner with participants through effective Care Management to achieve best outcomes in the delivery of quality clinical care and services.

  • Support and foster the ethos and values of West coast Homecare to enable positive workplace.

Qualifications & Experience

  • Minimum of 3 years demonstrated experience working in the health, in-home age care or disability support sectors.

  • Exceptional consumer focused customer service skills, with experienced in working in a fast-paced environment, always striving for excellence in the delivery of care and services.

  • Flexibility and a genuine commitment to getting the job done in a timely manner.

  • Leadership qualifications would be ideal.

  • Evidence of providing and enabling an excellent customer experience, and also providing exceptional internal and external customer service.

  • Proficient at using a computer including the application of client software programs.

Compliance requirements

  • Bachelor of Nursing, current registration with NMBA.

  • Satisfactory NDIS Worker Check.

  • Current SA Driver’s Licence essential.

  • Current First Aid & CPR certificate.

The Role

We are seeking a caring, reliable, friendly, competent and passionate Community Care Worker full time or part time in PORT LINCOLN.

Role Duties

  • Personal care – showering, toileting and grooming both male and female clients.

  • Domestic assistance - housekeeping and laundry.

  • Meal preparation and serving.

  • Medication prompting and assistance.

  • Transport – appointments, shopping and social outings.

  • Respite – being with clients to give carers a break.

  • Social support – assisting clients to access community.

About You

The successful applicants will benefit from having:

  • Exceptional consumer focused customer service skills, with experience in working in a home care environment.

  • Certificate III qualification in Individual Support, ageing and/or disability.

  • Current First Aid & CPR certificate.
